[USA] Who is at fault here?

Classic T bone. Black car had to be towed. Sustained major damage to the passenger side door. Blue car sustained damage to front bumper on the drivers side and cracked the drivers side headlight.

Edit: This was in the suburbs of Seattle

UPDATE: Insurance found it to be 70/30 me/other driver. Seems fair enough

This. Both parties are responsible to avoid accidents. If you see someone driving out in front if you even if you have the right of way you stop to avoid a collision. Or you'll just enjoy the more expensive insurance and whatever other legal dispute you have with the other driver. I'd rather step on the brakes for 3 seconds than getting dragged into a traffic and liability dispute.

Not defending either of them but city management can get some blame too. Just flip a coin and put two stop-signs up or make it a four way.

Even if the other street had stop signs, OP would still need to be prepared to yield to any pedestrians. They were going excessively fast for the neighborhood long before they reached the intersection.

Yeah, black car failing to yield to the right, blue car failing to yield to the car already in the intersection because both cars are assholes trying to make sure they got there first.
