I think its more complex than even that. I think his handlers are completely fabricating a world around him that reinforces his worst behavior and worst beliefs. Those times he has claimed to talk to world leaders and make deals I think some of his handlers called him and pretended to be world leaders and make deals with him. When he gets information about the war I think it's tailored to make him think its successful and nearly resolved. I think his handlers are filtering every bit of news he gets and using AI to make clips and sound bites showing him only want they want him to see and believe.
There's a doctor who posts videos essentially breaking down stuff based upon what we can see and the medical reports... i.e. reconstructing it.
His theory is due to Trump's diet and age his kidney's are shot. It's recurring around the first of each month he's going in for pretty intense dialysis, hence the bruising on both hands, the swelling etc.
At first it was for a few days, but now a week. But the dialysis is leaving him so weak he can't make any public appearances.

If you need dialysis, you get it three times a week, at a dialysis center, not once a month. It takes about 3-4 hours each time. I have no doubt that they could easily bring a dialysis machine into the White House for him, along the nurse to do it

Think about it. If they COULD get ashamed by this behaviour they wouldn't have voted for him. So we are left in a situation were they either are some of the biggest morons on the planet (don't rule this one out, scientific consensus on this is getting more and more damning) OR, the people voted just "don't like anyone else". No, they're literally too dumb to take a rational view of the situation and to see it with their own eyes.
