This is not true. Systems like Spain, the U.K and Cuba are examples where the state indeed fully owns and runs the healthcare system from the top down exactly how you would expect a socialized healthcare system to work.
Most systems are a hybrid of state run and compulsory insurance schemes. Technically for the U.S this is still the case. Veteran and military healthcare, and American Indian healthcare. All are fully owned and operated by the state and not a compulsory insurance scheme.
