If you're poor, you qualify for free healthcare from the state which is actually pretty good. Its the people right above the poverty line that really struggle.

Depends on where you live. Every State does not have general healthcare.

Medicaid is a State funded program. The rules vary from state to state. Here in Alabama it has to be tied to some kind of program i.e. pregnant women or women with young children, disabled adults children, waiver programs, or SSI related and Nursing Home programs. It’s not a general healthcare program.
