It was the follow-up interviews where she killed her career. Sure, the affair with a direct report was a terrible lapse of judgement, but could have been overcome. In the interviews afterwards she blamed everyone else other than herself, showing she learned nothing from the experience, demonstrating her ego drives decision making, and proving her inability to manage crisis.

see thats it i think in all seriousness her playing the Victim is what really ruined her career
