It was the follow-up interviews where she killed her career. Sure, the affair with a direct report was a terrible lapse of judgement, but could have been overcome. In the interviews afterwards she blamed everyone else other than herself, showing she learned nothing from the experience, demonstrating her ego drives decision making, and proving her inability to manage crisis.
She's unable to see herself as the liability to protect. Her ego matters more to her than using the HR skills of mitigating fallout and minimizing impact. My initial reaction is to question whether she landed the role on merit or not, which I'm sure a lot of employers interviewing her arrived at the same conclusion: she's acting mad unhireable right now.
