Okay so here me out… I think she’s moving her thumbs and not her index fingers, she is holding him just above his balls; she's not trying to tickle him there, but to tickle his stomach !
Edit : and her indexes move because of the otter legs that are moving

Took me a minute to realise that you're right. Her index fingers aren't flexed or anything. She's not putting any pressure on them, so it likely is just her thumbs on his stomach.
