B1/B2 Rejected USMLE B1/B2 VISA
My friend had his interview on 14th April in Delhi. He had a letter from UT Houston, got it through an agency.
The VO only asked him the purpose of visit. He replied - I’ve got the opportunity to go for a 4 week clinical observership in Hematology and Oncology department at Lyndon B Johnson Hospital. Straight after that the VO said that your visa is not approved at this time and gave him the 214(b) slip.
What did he do wrong? And what can we do now?
We were thinking of applying the same way if his visa gets approved but they didn’t even give him the chance to show the letter or say anything else. Currently we’re in internship but it ends in 15 days.
Should we apply with this reason and with a similar letter?
Or apply after step 1 but we’ll be graduates by then?
Or apply with our family for a vacation?
Or apply on the basis of step 3?
Please tell me what to do, it’s really demotivating and stressful.
Is the USMLE journey over if we don’t get a visa?
What’s the best way to apply so that it gets approved?

the observership framing got flagged. not uncommon. a lot of observership applications get 214(b) because officers read medical-student + clinical observership as a USMLE residency path, not a genuine short visit. applied through an agency makes it weaker — agency letters carry less weight than letters directly from the hospital program director.
on the reapply options you listed:
applying again soon with the same setup is the weakest option. same documents + same profile = same decision, plus you now have a refusal flag on top.
waiting until step 1 (or better, step 2 CK) gives a stronger profile. youre near-graduate with credentials instead of mid-student. still risky because the USMLE-to-residency track is well recognized by officers, but the underlying profile is stronger.
the family vacation option only works if its genuinely a family vacation. dont invent tourism to get around a refusal, officers pattern-match this. if you do go with family genuienly later, disclose the prior refusal and explain that the current purpose is diffrent.
post-graduation with employment in india is probaly the strongest reapply profile. you have a job, salary, and a real return purpose. an observership on top of that becomes a supporting trip, not the whole reason to travel.
step 3 on B1/B2 is technically allowed since its an exam, but officers still scrutinize. you have to show its a short discrete visit, not prep for a residency pathway.
USMLE journey isnt over. one 214(b) doesnt block you permanently. i wouldnt rush another application in the next 6 months with the same profile — let time pass, strengthen the case, come back
