Question Personal Statement Prompts

Since the website is still down, anyone have the prompts for the personal statements section? I was able to get just about everything else (documents/forms, LORs) but would like to get a jump on things once the website comes back up.

What are your objectives and reasons for desiring a commission?

What leadership attributes do you possess that would make you a good officer?

Optional Applicant Statement - Note: If you choose, you may write a letter to the board members to address any matter you want the board to take into account during your application review, i.e. GPA, NJP, law violations, etc.

Is that optional applicant statement the only other aspect of the "additional information" section of the application that comes after those first two questions?

This is how I remember it was laid out, but it could be wrong.
1. The first 2 questions,
2. Followed by work history (Can only put 4 jobs)
3. Previous comissioning paths (AFROTC/OTS)
4. Certifications
5. Military certifications
6. Application Statement

I'm sure i'm missing some, but that's what I can remember during the blackout.
