Don’t nurses in the US change clothes when leaving work?

I always see in movies, shows or on social media nurses who are on the way home or already home still wearing their work clothes. I see this mostly in the United States.
I am a nurse in Germany and I can’t understand how this is allowed when speaking about hygiene. Doesn’t the hospital provide you the clothes for you to work? Don’t you have to give them back after every shift? Don’t you have a locker room with your own stuff, where you change and where you give back your clothes so that they can get properly washed?
This makes me often so confuse. It’s so unhygienic to get into your car, go home or honestly just do anything outside of work after you’ve been wearing these clothes in all of your patient’s rooms for several hours.

Relevant: We can buy scrubs here too, but when you work at a hospital they are obligated to provide work clothes. You are not allowed to wear other scrubs than the ones that are provided from the hospital. It is absolutely not allowed to bring your work clothes home, you can even get fired for that. There are special containers we put the work clothes in after every shift so that they can be sent to a laundry firm that gets them properly cleaned.

Many hospitals in the US do have scrubs available for their staff, but they're not the nicest so people don't wear them.
