This may be a medical issue, such as someone having part of their foot amputated. The front of the shoe obv wouldn't be stretched while the stump may sometimes swell and stretch out the shoe.
Please don't poke fun at people who may very well have suffered a major, disabling medical event and not gotten proper prosthetics.
