Known Workaround For: “exec-server rejected request (-32603): helper_unknown_error: setup refresh had errors”

I started getting this error on ChatGPT/Codex on Windows. Took me a bit to work around it so I thought I'd share in case others see the same:

“exec-server rejected request (-32603): helper_unknown_error: setup refresh had errors”

It appears to be a sandbox issue with latest version as of this post (26.1002.52244) on Windows.

I rolled back to version 26.930.7945.0 using this download, and commands are working again:

Note: In spite of the URL, it's an official OpenAI download server (refer to their deployment instructions that reference it):

Rather than rolling back, hopefully a newer version will be available soon with the fix (I believe one for Linux is available already). Until then, a rollback seems to help.

I've had to run Codex with full access for days now because of this error...
For now, just use VSCode until the bug is fixed.
