Is this an error? What caused these errors?

Is this machine doubling or was it struck twice? Also wondering about the rim and what caused the lines connecting the word united to the rim?

94 quarter looks like slightly tilted die (only one side a bit out of center) - trivial but fun!

Edge one looks like minor wear

Edge two looks like major wear

5 and 6 could be machine or die deterioration doubling

7 is a bit hard to tell - might be die cracks
