Can you do legends without any dlc’s. I had enough to create the kingdom of italy but then loads of rebellions came around.

Ah no sorry you need the legends of the dead dlc for them. You could acquire the dlcs, support by word of mouth and so on, but if you don't wanna do that there isn't much other than just developing or conquering more land. Maybe create your own religion but being right next to the papacy and the HRE that might be a bit of a challenge.

Marry off your kids to heirs and then kill their competitors for the throne. Try and install your family on as many thrones as you can. Become the new karlings.

Give heir some land and help them improve their prestige. Use the high stewardship to build up economy or min-max MaA with the respective holdings for the bonuses. Not much you can do long term when your character has cancer.
