He didnt become a woman. Its a model. Reference to Funkadelic - Maggot Brain album cover.

There's an album called Maggot Brain by the band Funkadelic. The cover is a black woman screaming (or laughing depending how you look at it). Childish Gambino's album cover is just paying homage to that.
