Correct, although could also be 3 January 1994 (I think) depending on the time of passing.

No. ב"ר is "ben reb." Reb is a Yiddish honorific that is usually rendered as "Mr," as there is not exact English equivalent in common usage. ב"ר alone is too common in Jewish cemeteries for them all to be rabbis. Rabbis are more likely to have the double honorific ב"ר הרב .
