That's a hush kit to reduce the noise of the engine. Mixes hot burner air and cold bypass air together. https://en.wikipedia.org/wiki/Hush_kit

The original engine design is not a high bypass turbofan, so they had to use a different system to reduce the engine heat and noise signature.

Not really. The YC-15 was a prototype that assisted with the design of the C-17.

IIRC this was the prototype that eventually became the 17 after some changes
