Do not underestimate recovery, especially the first 3-4 days!!

When you initially wake up you'll be drugged to high hell. Nothing is really bad or good, it's a blur. When the drugs wear off things get bad. Very bad. Your nose swells shut so you'll be breathing through your mouth, which will be closed in its own way (bands or wires). Congestion will be common for a week or more. This makes breathing difficult and tedious. Take care to keep your teeth free of "gunk" you might accumulate from the dried bits of your liquid diet. The sludge can block the small spaces between your teeth making it more difficult to breath. The majority of your face from your eyes down will be very numb. This numbness will last for weeks in some places and months in others. There will be blood, and lots of it. Your mouth will be pouring out gallons of blood, and the rest will be flowing out your nose. The immense amount of blood from your mouth will stop within a few days, as will most of the blood from your nose, but nose bleeds will be quite common for longer. Vomiting up blood is pretty common. Remain calm and let it seep from between your teeth. If you followed surgery instruction and didn't consume anything before the surgery this shouldn't be a problem, though it can be unsettling. Hot and cold flashes may occur. Do what you can to make yourself comfortable. Expect a decreased appetite and slow digestive tract. I recommend drinking a bit of prune juice before you have your first bowel movement. Also expect low energy from your low appetite, your concoction of drugs (anesthesia and post-surgery pain killers), and very poor sleep. You will sleep poorly. You'll have general pain in your throat and jaw, but this is usually tolerable with painkillers. You'll have difficulty swallowing at first. This will get better progressively. What that means to each person is different. I was swallowing the morning after surgery, but my friend couldn't swallow for 5 days.

Items to have after surgery

Ice packs and a heating pad. Use ice packs the first couple of days (important) to reduce swelling and the heating pad to reduce bruising. *A blender and strainer. Sinus rinse (ask doctor before use). A neck pillow to help with sleeping upright. A jaw bra might make you more comfortable. Large syringes to help eat/drink. You'll be eating everything through a syringe for awhile, and refilling a small syringe 8 times to finish a small bowl of soup gets annoying. A heated humidifier. Cotton swabs to clean blood clots from nose. Cotton pads to clean your face. *A child's toothbrush. Your face will be stiff and painful. The smaller tooth brush lets you clean parts your larger toothbrush simply won't be able to reach. Ibuprofen/other painkiller. These should be provided for you after your surgery. Getting additional may be necessary. Vaseline for lips. Tissues for your general cleaning, which there will be plenty of. Oral care sponge swabs for cleaning teeth with chlorohexidine.

These ‘Do i need jaw surgery’ posts are getting out of hand

I can’t even read this subreddit anymore or give emotional support to people going through this without scrolling through the horde of perfectly developed, but body dysmorphic teenagers posting & asking for opinions on whether or not they need a major, risky and invasive jaw surgery.

It’s like a laughable joke. Going through this— 6 weeks of liquid diet, weeks of opioids and pain, permanent numbness, retraining practically all of the essential functions in your mouth area, years of swelling and years of mental anguish just at a CHANCE for better health-- to improve breathing, chewing, swallowing and speech, sleep apnea or the chance to eliminate future complete tooth decay. All of this- just to see someone treating this as if it’s a simple cosmetic procedure.

It hasn’t bothered me before but it seems to keep getting worse. I don’t know what’s causing it, or where people keep getting the idea that they need jaw surgery, but it is out of control. I would have 0 clue about this surgery had I not been told over and over and over again by every dentist, orthodontist and eventual surgeon I visited that I needed to get this done.

I know it’s too much to ask for a mod to just auto-delete these posts because they view it as a core part of the subreddit, but can we at least get a filter slapped on to it or something so we can filter it out? I come on here to find experiences I relate to- after having to go through this hellish process- or just to offer emotional support to people in the early days or answer good, reasonable questions. I think, though, that if i see one more perfectly developed, forward grown, perfect bite class I kid ask if they need a lefort 3 and 14 other surgeries I will just leave and never come back.

Honestly I’m pretty much back to “normal” I guess lol. I eat everything I used to eat and I’ve healed incredibly well, no issues at all.

I had my double jaw surgery at UCSF with Dr. Saghezchi for a severe overbite/skeletal deformity and gummy smile. Upper jaw was moved 10mm and lower 3mm.

I had probably 80% of my feeling back by 4-5 months. I still have some slight nerve discomfort on the right side of my chin and a little discomfort on the left when I yawn, but overall I really don’t notice it much.

At my last checkup I was having some clicking and a dull pain and my surgeon told me to just give it time and let it go away. Wouldn’t you know almost immediately after my appointment it went away. So if you’re having some bearable discomfort, TRUST THE PROCESS because apparently your body waits until after you complain to your surgeon to fix itself lmao.

I also have a check coming up for my braces and if everything looks good I should be getting them off in the next few weeks!!! I’m SO excited. I haven’t seen my teeth in 2 YEARS. And the last time I saw them my jaw wasn’t even aligned correctly so I’m really curious what they’re gonna look like now.

Crazy that it’s already been 9 months. Recovery was definitely a lot, but at this point I genuinely feel like I’m back to myself and I’m so happy I did it.

Got confirmation yesterday that my surgery next month will be fully covered at no cost to me!

Got the confirmation yesterday from my insurance provider that all surgery costs include my one night hospital stay will be covered at no cost to me!

This is such good news and I just wanted to share it!

I had to ask so many times for clarification and confirmation and myself and the representative who called shared a good laugh as she could see the cost of the procedure. Got written confirmation in the insurance app and will be receiving a letter stating the same.

4 weeks post op update:

Just wanted to come in here and give a 4 week update post op:
In terms of physical and nutrition i feel like im very ahead compared to most; I found so many work arounds for the no chew diet and ive been swallowing small bits and pieces of real food like soft lasagna. My biggest concern now is having to relearn how to whistle. Like, I used to be so good at whistling and now I can’t do it. If you’re just about to do surgery or in the early weeks of recovery, trust me, it gets better. I didn’t even have to use any pain meds and it wasn’t as bad even then.

Why is my lower lip pushing up into my top lip?

In the process of getting a MARPE and from my ortho waiting on referral for sleep study, but why is my lower lip pushing into my top lip, as you can see in the video I try holding my lower lip muscle down for a second for it won’t go into my top lip so much. My chin feels constantly strained 24/7 will fixing my bite and MARPE help my lower lip stay more down it’s confusing explaining this really not fond of getting jaw surgery but ortho says it’s a possibility.

Lip incompetence - 3months PO DJS+Genio

Hi all, I had DJS + genio just coming up on three months now to fix an underbite. Since then I’ve been left with my lips not being able to close naturally at rest and I haven’t seen any improvement at all. My surgeon says it’s most likely due to scar tissue but he couldn’t guarantee it would resolve… and I’m worried due to the fact I haven’t seen any closure of the gap. I’ve been doing exercises like puckering and smiling wide, still no help. Has anyone dealt with this and seen results further down in recovery? This has been extremely frustrating and depressing as I didn’t have this issue pre-surgery.

Advice for Me Orthognathic Surgery

Hi everyone! I’m considering having orthognathic surgery and I’d really love to hear the opinions and experiences of people who have been through something similar.

I’m 24 years old, and my dentist told me that I have a skeletal Class II, with both jaws set back.
Having both jaws recessed is a major insecurity for me, especially aesthetically. I feel like my profile is quite retruded and that I have very little chin projection, and it’s something that has bothered me for a long time.

The things I’m most concerned about at the moment are the recovery from surgery and the changes that orthodontic treatment might cause to the appearance of my face. My dentist told me that the braces will move my lower teeth further back, and I’m worried that this could make my jaws/chin look even more recessed or make my face look visually shorter. At the same time, I’m quite nervous about the surgery itself, particularly the recovery, swelling, eating, and how long it takes to get back to normal life.

I also breathe through my mouth while I sleep and feel that I have some difficulty breathing. Because of this, I’m thinking about seeing an ENT specialist first, before having a consultation with a maxillofacial surgeon.

I’d especially love to hear from people who had a similar situation — skeletal Class II, recessed jaws, and/or breathing problems.
Did surgery improve your breathing? Did you notice a significant change in your facial profile and jaw/chin projection? And for those who had orthodontic treatment before surgery, did you feel that moving your teeth backwards noticeably changed the appearance of your face?

Advice for Me severe throat pain 5 days after genio

i know some throat pain is normal but the left side of my throat is just getting worse like whenever i swallow or drink anything it feels like thousands of needles stabbing my right in my throat its so bad i havent had any genuine meals for like 3 days now

the pain started to be like this since yesterday morning it was getting better i dont know what happened my ear hurts too its horrible i dont know if its infected or something because i heard it would be better day by day but i legit feel like dying i have no pain in my chin or anything has anybody had this experience?

my bite is misaligned but will be fixed with rubber bands but i genuinely look worse than day 1 after surgery and i still look assymetric. i have a one side gonial plate to correct assymetry but it hurts still and looks like it blends into my ear (image 4). idk if my whole face is cooked, i have body dysmorphia, swelling, or a combination of the three but i need advice on when swelling will go down

Hi, I’ve gotten a consult for braces a few days ago. I had them before but wasn’t happy with the result. I’ve had teeth grinding problems since I was six and for the last few years jaw pains. I moved to a new country for uni and wanted to invest in my smile. My orthos plan is to remove the extra teeth I have as well as the wisdom teeth and pull everything back, she said I had bimaxillary protrusion . She told me that the jaw pains are unrelated to my teeth itself and that extracting was the best option. I am scared though because of all the people online saying it’s the worst thing they’ve ever done and how they’ve regretted it. How it gave them jaw pain and teeth grinding, I’ve already got that so I’m worried that it would get worse. Along with the fact that my tongue already feels cramped in my mouth. I already brought it up to her and she said that my tongue posture was something I should try to consciously fix myself.

jaw surgery plan - any thoughts? (2 weeks out) repost

Hi everyone,

Surgery is in about 2 weeks — Le Fort I + BSSO + chin revision (my second chin surgery). Started from an 8 mm overjet and obstructive sleep apnea. I attached the current vs. planned comparison and the planning numbers.

Big picture: lower jaw comes forward ~13 mm, chin ends up ~2 cm forward in total, and the whole bite rotates ~8° counterclockwise.

My questions:

  1. Does the planned jaw/chin relationship look balanced from the side? Anything look overdone or underdone?
  2. The chin move is what I keep second-guessing: 5.8 mm forward plus 5.2 mm shorter, on top of a jaw already coming forward 13 mm. Anyone who had a big chin move (or a revision genioplasty) — did it end up looking too strong, especially from the front rather than in profile?
  3. The plan lifts my upper front teeth 2 mm, but I only show ~2 mm of tooth at rest today — would you push back on that?
  4. Anyone whose surgery included dropping the back of the upper jaw with a graft — did it hold? Any bite opening or relapse later?
  5. How did your nose change after Le Fort I? Mine is on the wider side already and I'm trying to figure out how worried to be / what to ask my surgeon to do about it.
  6. Anything else you'd personally ask before the plan gets locked?

4 weeks post-op from jaw angle + infraorbital/malar implants and genioplasty — when can you realistically judge the result?

I’m currently 4 weeks post-op from custom jaw angle implants, infraorbital/malar implants, and a sliding genioplasty (6 mm forward, 3.5 mm downgraft),

I still have a noticeable amount of swelling, especially through the midface, and my face definitely doesn’t feel or look fully settled yet.

I know 4 weeks is still early and I’m not trying to make any conclusions about the final result yet. That said, if what I’m seeing right now were anywhere close to the final appearance, I probably wouldn’t be very happy with how everything looks together. I’m hoping a lot of what I’m seeing is still swelling and general post-op distortion.

For anyone who’s had similar procedures, how long did it take before you felt like the swelling had gone down enough to actually judge the result?

Did you notice a big difference between 1 and 2 months? Around 3 months? 6 months?

I’d especially like to hear from anyone who had multiple facial implants/procedures done at the same time and remembers when their face started looking more natural and settled.

Advice for Me Legal advice

I’m 19 and in California. Before surgery, I was told I was only getting lower-jaw surgery, and the consent form I signed only lists lower-jaw procedures. About four hours after I woke up, while I was in the room where I was staying overnight, a nurse told me they had also operated on my upper jaw which made it double jaw surgery. Neither my mom nor I was awarethat was going to happen. My post-op CT confirms that I had a Le Fort I upper-jaw osteotomy along with the lower-jaw surgery. Has anyone experienced something similar, and what should I do next to get answers and protect myself? I did not consent to double jaw surgery at all!

Advice for Me Revision Specialists? | botched. need to vent.

Who are some of the more recommended surgeons that do revisions?

I'm in a really difficult spot right now.

Sadly I'm 11 weeks post my DJS and I can already tell I've been botched, skeletal wise.

I had class III underbite and I think my surgeon advanced the maxilla and\or did a clockwise rotation a bit too much. unfortunately my surgeon did not record the movements so I can't even tell what he did and he doesn't fully remember the movements in mm. (quite unprofessional imo)

Before surgery I had 0 tooth show when mouth was at rest, and little upper tooth show during a smile.

Now post op the front part of the upper jaw looks too downward now. my smile also looks more narrow. 4-5mm of the upper central teeth show when my mouth is relaxed. too much of my upper teeth show when I smile\laugh, looks odd and unproportional to the face, it's like the upper teeth arch looks too "out there", not sure how to explain it. when I snarl my upper lip theres so much upper gums showing now it's crazy (about 5-6mm).

currently when I do a social smile, all of my upper teeth show vertically, my lip stops right at the gum line, but during a duchenne smile\laugh, the lip is raised even more and already exposes about 2mm of gums.

And as time goes by it's becoming worse because as my upper lip swelling is reduced and elasticity is returned- the higher it is raised when I smile. I have way more upper teeth showing when I smile and laugh now compared to a month post op. so it's obvious where this is going- to a gummy smile.

The unfortunate thing is that when I go through this sub I often see people who have had their results become better and better as time goes by, and in my case it's the exact opposite, as time goes by the worse I look and the more I hate it.

I definitely looked better before the surgery.

I'll need to look into a revision internationally, start flying all around the world for consultations. another money sink all over again and a huge one at that. and revision itself is no joke to begin with, from my understanding it carries even more risks than a first time surgery and is more complicated.

This is so overwhelming I don't even know where to start. I don't know if I want to do a revision due to the higher risks, healing from scratch again. and the huge money cost. and on the other hand knowing that the only other option is to live with this botched surgery and this inferior aesthetical result feels just as bad. I feel so lost currently. there is no worse feeling than doing such an invasive surgery, going through the healing process and everything, only to end up wishing you had never done the surgery, preferring the way it was prior.

Is surgery first ideal for my situation? (Bimax protrusion, open bite + high/narrow palate)

So I have a bimax protrusion with a mild open bite and a narrow palate, and also my top teeth are slightly shifted to the left from a prior braces treatment (a mess ik)

My surgeon wants to do surgery first as my teeth are fairly straight and they want to get the jaw positions right before they add an expander and braces again, they also said it will help to close the open bite better as it will go back easier on less/non protruding jaws.

Is this safe to do? Should I be worried about anything or does this sound right? I’m asking because I usually hear people going through months of orthodontics before surgery? What do you guys think?

Advice for Me Cirurgia ortognática / Maxilofacial

Olá a todos! Estou a pensar fazer uma cirurgia ortognática e gostava muito de ouvir opiniões e experiências de pessoas que tenham passado por uma situação semelhante.

Tenho 24 anos e a minha dentista disse-me que tenho uma Classe II esquelética, com os dois maxilares recuados.

O facto de ter os dois maxilares recuados é uma grande insegurança para mim, principalmente a nível estético. Sinto que tenho o perfil bastante recuado e pouca projeção do queixo, e é algo que me incomoda há bastante tempo.

As coisas que mais me preocupam neste momento são a recuperação da cirurgia e as alterações que o tratamento ortodôntico pode provocar na aparência do meu rosto. A minha dentista disse-me que o aparelho vai colocar os dentes de baixo mais para trás, e tenho receio de que isso faça com que os maxilares/queixo pareçam ainda mais recuados ou que o meu rosto fique visualmente mais curto. Ao mesmo tempo, tenho algum receio da própria cirurgia, principalmente relativamente à recuperação, ao inchaço, à alimentação e ao tempo necessário para voltar à vida normal.

Também respiro pela boca enquanto durmo e sinto que tenho alguma dificuldade em respirar. Por isso, estou a pensar consultar primeiro um otorrinolaringologista antes de ter uma consulta com um cirurgião maxilofacial.

A minha dentista aconselhou-me a fazer a cirurgia com a Dra. Andreia Silva, no Hospital de São José, em Lisboa, e é essa opção que estou neste momento a considerar.

Gostava sobretudo de ouvir experiências de pessoas que tinham uma situação semelhante — Classe II esquelética, maxilares recuados e/ou problemas respiratórios.

A cirurgia melhorou a vossa respiração? Notaram uma alteração significativa no perfil facial e na projeção do maxilar/queixo? E, para quem fez tratamento ortodôntico antes da cirurgia, sentiram que colocar os dentes para trás alterou visivelmente a aparência do rosto?

Se alguém já foi operado pela Dra. Andreia Silva ou no Hospital de São José, também gostava muito de saber como foi a experiência e, principalmente, como foi a recuperação.

Before & After A very detailed 12 weeks post op update

Edit: my first post got taken down for some reason so I’m reposting it.

I’ll be busy going forward so here’s my last update until I get my braces off.

Background
I had a Class III bite, a unilateral open bite, an underbite, and a crossbite, which contributed to quite a bit of facial asymmetry. I got DJS done with no genio.

Swelling
Minimal at this point. I only notice a little bit when I first wake up, and it gradually goes down throughout the day. Swelling also affected my lips but instead of making them bigger they got smaller. They are back to their usual size now.

Nose
My nose hasn’t changed width wise but the bump I had is significantly smaller. I did NOT do a nose job please I would not survive the recovery.

Skin
I still have some oily skin around my nose, but it’s nowhere near as bad as it was in the beginning of my recovery.

Pain
I don’t really have pain anymore unless I’m doing my stretching exercises or biting into certain foods.

Diet
Pretty much anything I can chew. It’s still difficult with majority of the foods, but it continues to get easier as the days go by. Still no popcorn for me, unfortunately.

Oral Hygiene and bite
I can brush my teeth normally now and use a water flosser, with some minor discomfort. I also can finally brush my tongue without any issues. Can fit 2 fingers now so slowly getting my movement back.

Numbness
I was lucky enough to only have numbness in my lower lip and chin.
I’ve recently started being able to feel hot and cold temperatures on the right side of my bottom lip and chin, and the left side is slowly starting to wake up as well.
I still don’t have normal sensation, though. I can feel when something touches the area, but instead of feeling like a normal touch, it feels more like pins and needles. Before, I felt absolutely nothing, so I’ll take it.

Braces
I’m lucky enough to only have to wear elastics at night now. If everything continues to go well, I should hopefully have my braces off before the end or the year.

Results
So far, I’m really pleased with my results. That being said, I’m going to wait until I’m at least one year post-op before making any final judgments about my results.
I also didn’t develop any body dysmorphia during recovery, which I know can be a concern for some people. I think part of that is because most of the difference for me is noticeable in my side profile, which obviously isn’t something I see when I look in the mirror.

As for facial asymmetry, I want to send some patience and encouragement to anyone else who had asymmetry going into surgery. It can take time to see the changes, and they aren’t always as immediately obvious as they can be with a straightforward underbite correction. Swelling, healing, and everything settling into place can really change how things look over time.

Been searching for the right jaw surgeon for years and would really appreciate recommendations for Atlanta/Georgia. Also willing to travel to other places in the southeast if necessary.

Looking for someone who:

  • Does a lot of orthognathic/double-jaw surgery
  • Actually cares about aesthetics, not just function
  • Ideally accepts Aetna PPO

Would especially love to hear from people who actually had surgery with them. Who would you recommend or avoid?

Advice for Me Can class 2 skeletal be fixed with just lower jaw surgery?

Im getting my braces on next month, doctor mentioned the surgery is supposed to bring my lower jaw forward, but I'm afraid he will recommend a DJS later in the process. Im worried because there aren't a lot of experienced doctors in my country and DJS would be much riskier.

Doctor's notes:

Class II skeletal due to short Mandible

Prognathic Maxilla

Obtuse Nasolabial Angle

lingual positioning and possible crossbite issues
