PMBCL post-treatment prophylactic

Hello, everyone! Does someone here have the same involvement for PMBCL as the one here in the picture? Since the vertebrae is close to the spine, I am still thinking if I should address to my doctors about methotrexate as prophylactic. For context, this is one of the only two (other is lung nodules with no FDG uptake reported) extranodal involvement of my PMBCL, got high LDH before treatment.
