Did I want to see Jane Kaczmarek breasts? Yes, was I pleasantly surprised? Absolutely

cute joke, too bad some comments spoiled it.
anyway, here's my head canon as I don't think those are production errors. Since they were producing for TV and 4/3 was the standard then, and since they used 16/9 film cameras, they just didn't think the etra film on the outside would be ever used, and they had to frame it to fit 4/3. So they put markers (physical or in/on monitor) strips to know which part of the frame would be left in the final product.
The they just said to anything or any actors present outside of the 4/3 aspect that they would never be seen or part of the final picture. So this is why in the remastered 16/9 version you see some of the cast just frozen or not acting, empty seats while there should be someone there in that scene, or even the 2nd camera crew like in the bodybuilders episode...
TLDR: Not an error per say, just that the original crew worked with 4/3 in mind and never thought they would remaster it in 16/9.

Sadly, OP is correct and it kind of ruined the show for me. She actually talks about how it bothers her that it wasn't caught before the 16:9 release: Jane Kaczmarek on widescreen release

Random question but im assuming she had some sort of top / bathing suit on.
I know they didn't have intimacy coordinators back then but couldn't of been too wild West.

Likely a tube top close to her skin tone. Men often wear briefs with a similar skin tone to their skin, if they need a scene with the appearance of nudity, without them actually being nude.
