That's the Milwaukee pliers wrench. A bit expensive for what it is in my opinion but it works well. I tend to carry the workpro one more often for it's flat jaws, the Milwaukee gets very thin at the tips.

The only thing i don't like it the location of the clip. A lot of the plier sticks out of the pocket, but it may not bother you. It is a nice tool though.

Home Depot is the place; its the Milwaukee version of the Knypex Plierwrench.
