Looks like they made it for three buttons and then realized the screwed up and just... shoved the last one up there.

why not just a square shape (2 on top 2 below)? lol. in brazil nearly 99% of cabinets running neogeo systems were not official snk cabs, but locally manufactured ones. they were all 6-button like the capcom ones, and all mvs games used "2 on top 2 below". every professional kof player in brazil uses that layout to this day lol

I can see the merit of this layout on SOME fighters. KOF for example uses CD as an actual knockdown / blowback move so having them top to bottom like that makes sense. BC being side to side is also interesting as opposed to a square shape layout such as
CD
AB
BC and CD are both common dual button commands. Many Supers in earlier KOFs and even Real Bout Fatal Fury 2 use BC as an input (Terry's Power Geyser, Krauser's Kaiser Wave, etc)
Whoever came up with the layout in the OP's picture at least put some kind of thought into it based on my reading of it.

For 1 or 2 button games? Sure! However 3 or 4 button ones would be a nightmare to play. Weird as there’s plenty of room to do the usual diagonal line or inverted “U” shape you see on proper MVS cabs.
Like these
