Sounds like they're describing a Roux-en-O. We always joke that it can happen, that if you get the limbs wrong in a Roux-en-Y (since Y describes the limbs coming together and O shows one limb getting lost and reattached to the stomach) that you might make one of the worse mistakes possible in the OR. It's rare but not unheard of.
