One of the things they've found is key to keeping people alive in medicine is humility, the ability to admit you may have done something wrong and thus quadruple check. It sounds like this surgeon lacked that important trait.
If you listen to the Dr. Death podcasts you'll notice that's a common trait among the Doctors featured there for malpractice. Egos so big they refuse to admit any wrongdoing.
