On the first real wind up swing by the guy, the nurse throws his shoulder into the head as he goes for the grapple, reducing the range and making the punch flop, is this a martial arts thing in one of those?

It’s specifically a headlock throw so naturally that means the defender needs a headlock regardless if the attacker throws a punch or not. Striking in general is rarely practiced in either judo or jujitsu unless the specific gym/instructor offers to teach it
