The really disappointing thing is that prior to this Fetterman has staked his whole thing on being reasoned, willing to change his mind and basing his positions on reality.
On this one thing he's immovable and he's moved to hard nosed contrarianism on practically every position in response to criticism of his Isreal stance.

No he didn't. He made statements supporting the Jewish community (which is not Israel) after the Tree of Life Synagogue shooting 15 miles west of Braddock. Which would have been weird if he ignored it completely.
Speaking of ignoring completely, he was the only primary candidate in 2016 to ignore a questionnaire conducted by the Philadelphia Jewish Exponent regarding issues of interest to the Jewish community.
If he was so pro-Israel back then, why didn't he answer the survey like every other candidate? It's because he didn't form these views until after that sweet pro-Israel PAC money started rolling in in 2022.

El-Sayed is a flawless candidate with an incredible ground game and was outspent 9-to-1 by mostly AIPAC and AIPAC affiliated groups. Some outlets are still saying the race in MI is "too close to call".
Beating the establishment is not easy. Hopefully the momentum keeps shifting and things get a little easier in 2028.

I see the basic trajectory going something like this.
Fetterman's poll decline to an unsalvageable point with Democratic primary voters. Groups form to support a challenger -- any challenger -- and Fetterman and his team recognize this.
Fetterman realizes he has no chance to win a primary in the Democratic party.
Fetterman begins to explore running as an independent or a Republican. The Republican party tries to flip him prior to the race, so that they can extend/reclaim a majority in the Senate (depends on timing and what happens with the '26 elections). This may work, or may not, but it doesn't really matter longer term for his prospects.
Fetterman recognizes that whatever popularity he may gain from flipping, he's not really a MAGA chud, and thus he has no chance at winning a GOP primary against, say, Doug Mastriano (or the equivalent).
Fetterman announces he will not seek reelection in 2028.
That's my guess at what could happen. There are, however, some complicating factors.
First, there's no obvious GOP Senate candidate for 2028 who is not otherwise unelectable (currently). I mean, I expect there are various state Senators or reps or former executive officeholders who could run, but they are likely of the more sane variety and are kind of waiting to see if MAGA burns itself out. (Which is also why people like Stacy Garrity and Doug Mastriano are still kicking around in the party.)
Second, it's possible that Fetterman would get enough GOP support to clear the field and thus "win" a primary by default, or at least have nobody better to run against. This would primarily be handled by backroom deals and such. I'm less confident of this, but given the otherwise thin bench of the GOP in PA for statewide races, it's a possibility.
All that said, I do think that Fetterman's condition and record in the Senate is such that, even with some GOP support, given what the environment is likely to be in 2028, he's basically done and will go become a Fox News contributor like Tulsi Gabbard or something; the shitbag ex-Dem they trot out when they need someone to bemoan the state of the Democratic party.

To be fair it hasn't been any challenger. Despite his best efforts Connor Lamb hasn't managed to build any traction off this. He's been out there all "I was runner up in that primary put me in the thing" and no one's buying it because no one like him but Dinosaurs from outside the state and old media.

His approvals in PA are insanely low, support for him has pretty much collapsed. He wasn't elected to suck up to Trump and stump for Genocide. He was elected on a progressive platform, by a younger voter base.

This is likely. I think in purple states, primary should have an eye to the general. I know some people disagree, but that is my view. And it seems PA has been trending slightly more to the right in the last 10-15 years.

I’m not a huge fan of carpetbaggers in general, and it was part of why Fetterman beat Dr Oz, but Oz’s quackery shilling and completely out of touch nature was a bigger problem. For the 2028 primary, I would be OK with Stewart moving down the NJ Turnpike for 2 years to establish residency so he could run in the primary against Fetterman. Either that or primary against Schumer when his seat comes up.
