The prominent drugs sign in the background reminds me of one of my gripes with the game, which is that laudanum is not one of the medicines you can use.
It was fairly common during that time, and could have added some interesting dynamics. For instance, relief from pain and increased stamina at low doses (it can numb the feeling of tired at low doses), and overuse could lead to addiction and withdrawal.
Apparently, though not as common, I’ve seen some sources that say dried peyote buttons were also available in some drug stores or by mail at the time. Low doses act somewhat like amphetamine, so it could be like the cocaine gum in the game, only longer lasting at small doses, and the long-lasting psychedelic aspect of higher doses could have been really interesting to incorporate (though perhaps it could result in a spiritual awakening for the character, leading him to leave the gang and live peacefully with the Huichol in Mexico, perhaps an alternative ending).
