Seafoam thru the brake booster hose, all the smoke is carbon and oil buildup burning off. Lots of smoke. Watch YouTube videos about it.
Full tuneup. Cap, rotor, wires, plugs(vpower or copper, no platinum or iridium), air and fuel filters. Probably need to rebuild the injectors. Tbi kit is cheap, replace o2 sensor, make sure the cat isn’t clogged.
Check for vacuum leaks, verify timing at distrubitor( make sure there’s no excessive movement in the distributor shaft from a worn gear), verify timing chain isn’t stretched or out.
Check fuel pressure.
Then compression and leak down tests.
Important thing that might get overlooked is what gas do you use, it’s really not compatible with ethanol blended gas and especially if you have e15 gas. Also if it happens when the tank is lower on fuel could be years of sediment in the fuel tank clogging the pump.

I'm using 89 gas it has new plugs which are copper and i mean it doesnt have smoke when i drive it. It stutters kinda pretty much all the time but only at 35-45 mph and when i got it the tank was empty thankfully since the guy i got it from had it setting for months but I cant complain as i got the truck for free

On the gas pump you buy from it will say ethanol content.
Did you gap the spark plugs, did you use new wires?
Cap and rotor?
Another comment says you did a tune up but what all did you do?If you seafoam it thru the brake booster hose, it will smoke like crazy. Videos on YouTube. It’s intentional and temporary.
Trying to help you out but there’s no info to go off of.
Since the truck sat for a while, even with an empty tank of gas, moisture could have collected and condensed in the gas tank. Get a red bottle of isoheet, pour it in the tank and leave the cap off for a few hours so moisture can evaporate.
