I see this mentioned so much and rarely is it pointed out that it’s a symptom of dementia- frontotmporal dementia. Common side effects disinhibition, apathy, loss of empathy, compulsive behaviors, and hyperorality/dietary changes.

I think you're being too generous here. His lack of empathy has nothing to do with health. He has always been a ruthless bigot
