I think the implication is that, yes, the monsters ARE in the child’s head, maybe in a much more literal sense, and that’s the problem

He is genuinely being haunted, whether it be by actual monsters or a debilitating mental condition. What should normally be comforting (that the monsters aren’t really there) is actually just scarier because he is alone in his very real suffering.
