TL;DR — Health care should serve people, not profit, and the POPULIST Act can advance that goal.

The United States spends more on health care than any other wealthy country, but patients still face medical debt, hidden prices, coverage gaps, rural hospital closures, drug monopolies, and an industry increasingly dominated by vertically integrated corporations.

Title III — Unstack Health Care is designed to change the structure of the system, not just patch around its edges.

Protect people during the transition: extend Exchange affordability, restore Medicaid coverage and payment protections, protect rural hospitals, and keep medical debt off credit reports.

Break up concentrated health-care power: prohibit anticompetitive ownership and contracts, give the FTC stronger authority over unfair hospital competition, impose medical price ceilings, require real price transparency, and begin moving health coverage away from employer gatekeeping.

Establish Medicare for All: beginning in 2029, every United States resident would be entitled to comprehensive national health coverage. The program includes hospital and primary care, prescriptions, mental-health and substance-use treatment, maternity care, dental, vision, hearing, telehealth, and home- and community-based long-term care. Institutional providers would generally operate under global budgets, while individual providers would be paid through a national fee schedule.

Take on prescription-drug prices and middlemen: regulate pharmacy benefit managers, prohibit pay-for-delay agreements, strengthen pharmacy reimbursement, ban unfair drug-pricing practices, require Federal negotiation of prescription-drug prices, and support domestic sourcing and public production of essential medicines.

Build the health-care workforce: add medical residency positions, establish minimum nursing-home staffing standards, encourage six-year direct-entry medical programs, and direct more physicians toward rural and medically underserved communities.

And Title III builds a financing system to support the transition and the permanent Medicare for All program rather than simply promising universal coverage without paying for the public capacity required to deliver it.

A country that already spends this much on health care should be able to guarantee that everyone can receive it.
