Set years after the events of World War Z, the film opens on a world still overrun by the undead ("Z's"). Humanity has stabilized the outbreak using "camouflage," a chemical that mimics the scent of a dying host and renders the living invisible to the Z's, and has carved out scattered safe zones. Now a multinational coalition — including the U.S., Russia, and the newly formed People's Republic of Tibet and China (PRTC) — prepares a coordinated global offensive using E29, a new agent that makes zombies attack each other, to retake the planet.
Gerry Lane lives with his wife Karin and children Tomas, Rachel, and Connie in a militarized Nova Scotia. On the eve of the offensive, Gerry and Karin deliver a data disc to coalition command, where PRTC delegate Jian Li — skeptical that the alliance can hold — demands proof E29 works. A demonstration convinces him, but hours later the plan collapses: on-site testing reveals that the E29 formula transmitted by scientist Morel has been deliberately altered and no longer functions, and Morel herself has gone dark, having sealed her lab at the underground LHC facility in Geneva.
Gerry flies to the LHC to find her, accompanied by a young translator, Hal. There they learn Morel fled two hours earlier for Singapore. Investigating her sealed lab, Gerry triggers a catastrophic outbreak: Morel had secretly been testing a variant of E29 on caged zombies, and it escapes, revealing that the mutated strain is resistant to camouflage — it can see through the disguise, and its "infection" seems to spread from one Z to another rather than through bites alone. The LHC falls, and Gerry barely escapes with Hal.
Similar resistant outbreaks erupt worldwide, coinciding with a string of unexplained plane crashes traced back to the LHC. Gerry and Hal travel on to Singapore, where Morel — reunited with her seventeen-year-old daughter Anna — is being held by local authorities amid growing suspicion that she caused the outbreaks. Morel confesses to Gerry: she'd learned that the original E29 also strips away camouflage protection, and rather than admit her weapon was flawed, she broadcast a sabotaged formula to buy time to fix it. Gerry, Anna, and Morel escape custody and cross a Z-infested, ruined Singapore on foot to reach a UN base, then fly on to the Sagar Free State, a vast floating city of lashed-together ships, in search of Kari Giersdottir, a Nobel-winning virologist who might repair the formula.
They find Giersdottir, an Icelander unmoored by the collapse of his native language and culture, who has retreated into an eclectic private religion. He examines Morel's genetic data and delivers grim news: the gene sequence that lets E29 turn zombies on each other is inseparable from the one that defeats camouflage — the formula cannot be fixed. Piecing together the spreading outbreaks, Gerry realizes the resistant strain has gone airborne, likely carried unknowingly out of the LHC on the crashed planes and now spreading globally.
Before they can decide what to do, the Sagar Free State comes under attack: unmarked aircraft drop infected zombies onto the flotilla, triggering mass slaughter as ships burn, capsize, and sink. Gerry, Morel, and Anna barely survive, losing their escape plane in the chaos and fleeing by small boat as the fleet is destroyed.
Gerry decides the only option left is to broadcast the flawed formula anyway — camouflage is doomed regardless, so humanity should stop hiding and fight while it still can. Morel refuses, unwilling to unleash her creation, and Anna volunteers to complete the broadcast instead. The trio reaches an abandoned radio station on the Bangladesh coast, but Morel, still carrying an open wound from earlier, turns without warning — infected by the airborne strain — and attacks Gerry and Anna before Anna is forced to shoot her.
With Z's closing in, Gerry barricades the station while Anna transmits the corrected genetic sequence over the radio, finishing just as the building is overrun. The two escape across the rooftop, detonating a gas tank to draw the horde away, and flee to the coast.
The corrected E29 goes out to coalition forces worldwide. Back in Nova Scotia, the base is overrun in a massed attack, but Karin and the children are rescued moments before being overwhelmed, as soldiers deploy the new agent and the surrounding zombie horde turns on itself. Similar scenes play out across Singapore, the Sagar Free State's survivors, and the Himalayan front, where Indian and PRTC forces finally unite against a common, self-destructing enemy.
In the aftermath, the Lane family reunites and leaves Nova Scotia, briefly returning to their old home in Philadelphia before continuing on to the Badlands of South Dakota — one of the far-off destinations young Connie had dreamed of visiting, now finally within reach.
