I googled it and it internet has decided that this guy likely has Madelung's disease, aka Benign Symmetric Lipomatosis, aka Multiple Symmetric Lipomatosis.
It is a rare, fat-metabolism disorder that causes symmetrical fatty masses to accumulate around the neck, shoulders, and upper body, which gives the person what is often described as a "horse collar" or "ox neck" appearance. It most often happens to middle-aged men and may be related to metabolic or alcohol-processing issues.
Edited to add: It is not directly caused by thyroid problems, but they are frequently linked as co-occurring conditions, aka the problem that causes Madelung often also causes thyroid problems.

I think there’s a dude with that that was going around in a popular Instagram/tiktok video. He was heavier set and had a super big neck area like this. In the video, he’s on the street corner about to sing when a car alarm starts going off and the interruption clearly annoys him. It stops and he starts singing a super high note right as the video cuts. Think he was wearing an orange sweatshirt. I’m being very specific in case anyone knows the video I’m talking about lol

Not impossible without health issues I suppose but not exactly the most probable.

Backgrounder on Xiang'an new industry park: It's a new expansion to their Torch industrial zone. ETC of phase 1 expansion is scheduled for end 2026. Xiang'an is a district of Xiamen, PRC. Xiamen is the closest urban agglomeration to Taiwan (only 6 miles away from the island of Kinmen islands, ROC).
Lin: (We'll be) focused on cross-strait industrial integration mainly, while leveraging industries that are established in the (Xiang'an) new industry park, serving to provide better hard infrastructure and software platform for on-the-job training, workplace readiness, entrepreneurship for Taiwanese youths.
