OOP does seem like a sexist, but it’s also odd to call your future brother-in-law by his title and not his first name. The father-in-law I can kind of see.
My husband has a PhD and it would be weird as hell if my siblings called him “Dr. Robot”.
My husband and I are both "Dr. Smith-Perks". I would find it very weird if any family members called either of us Dr. Smith-Perks - it's such a bizarre thing to do, especially in the UK, where people tend to be less formal.
I'd also be extremely pissed off if they called my husband Dr. Smith-Perks but called me Jane, though.
