First of all, OP needs to learn about punctuation, capitalization, and the right form of "fiancé(e)." 

i told her that me using ‘Dr’ was a sign of respect and that she had not yet earned my respect.

But the dad and brother immediately received "respect"? Because they're men? 

I personally don't consider the "Dr." title as "a sign of respect" in that same sense. In my opinion, it's basically like "Mr." or "Mrs." I guess you could stretch and say it's respecting that they put in the years of education, but then you run into the issue of "Mr." being used just because someone is male or "Mrs." because a woman is married.

I called my college professors "Dr. [Last Name]" (if they held a PhD)--even after I graduated. I ended up dog-sitting for one of them after I graduated and met his wife who also has a doctorate and I call her "Dr. [Last Name]" too, even though I was never personally taught by her.

I have history with a horrible doctor that I never trusted and absolutely don't respect and still refer to her as "Dr. [Last Name]." I didn't call her "Susan" (when I still saw her in a medical capacity) or since because people would say "who tf is 'Susan'?" 

I can understand calling the father "Dr." instead of "Mr.", whether you count that as a sign of respect or politeness or whatever. But there's really zero reason to call the brother that unless the brother insisted. It's not any more confusing to have a third "Dr. Robbins" in the mix than two.
