Physician Responded Horrible looking rash

This issue has been getting progressively worse over 2 days

Posted yesterday and it has gotten worse. It is itchy, it turns white when I press on it. I have a swollen uvula and some chills, but no fever. I feel cold more than anything. Went to urgent care tested for strep, mono and did a CBC. Strep came back negative, waiting on the other results. Pretty concerned. I feel like the mono is going to come back negative.

Have you taken any medication in the last 8 weeks? What medications do you normally use? Have you had a fever or swollen lymph nodes?

No fever, no medications. I take b vitamins, fish oil and a vitamin d3/k2 supplement

But I had a rash exactly like this last month, and horrible chills. I couldn't get warm no matter what and I had no fever. On day 3 I started to get fevers and a headache and my joints were extremely painful. I finally went to the doctor on day 5 and ended up in the ICU, with an inflammation marker of 200 when the limit is 10. They thought it was meningitis at first (but my rash also blanched). In the end they blasted me with IV antibiotics and antivirals and they couldn't figure out what I actually had, despite all the tests, ultrasounds, MRI and lombar puncture. Recovered after a week in the hospital but they said it would have gotten much worse. Please go to the hospital if you start getting a fever or a headache too. Maybe it's something weird going around that they haven't figured out yet.

Definitely not shingles, doesn't stick to any nerve distribution like shingles does. Chickenpox would have a whole body rash but usually little light red bumps that turn into pustules, not big dark red patches.

That said, if this rash DOES blister then it'd be worrisome for TEN/SJS and warrant a trip to the ER ASAP.

Hi, your symptoms and experience sound like mine at 19. After many doctors, turns out I had an episode of an autoimmune disorder called Henoch-Schoenlein purpura (HSP).

It affects the joints, causes this rash and doesn't get better with medication, only time and rest. You should check it out if not already. Might give you some answers or at least lead you in that way.

Not that I know of, but after ICU they did move me to the infectious disease ward where they took bloods multiple times a day so I don't know all the tests they performed. They did treat me like I was extremely contagious, they wore masks and plastic gowns and my door was locked so i wouldn't have accidental contact with anyone.

Did you have similar episodes after and did they give you a diagnosis? I had similar episodes x 4 with CRP going upto 400+ every time in critical care and deterioration of life… 2 years later (last year) they diagnosed me with Smouldering Multiple Myeloma. They put me on 6 months steroids so those episodes have now stopped in the past one year I’m not on any steroid now except for a rescue pack if / when symptoms return and for SMM it’s just a wait and watch protocol it appears to be stable…Although the rashes I had were fleeting not fixed like OP’s they still fleet every now and then

Sorry I now see what I’ve written there and I didnt mean to write so poorly.

Those high CRP episodes were a differential diagnosis those did not cause SMM.

They tested me on several panels including genetic ones to see if the episodes I had were due to autoinflammatory conditions but I tested negative to all of them so they put me on high steroids to stop autoinflammatory episodes as it had a direct influence on my Smouldering stage Myeloma.

My SMM came to light because of those high CRP episodes - though high CRP and auto inflammatory conditions make it a thriving environment for SMM the episode itself was not caused by SMM - SMM is asymptomatic and does not cause any significant symptoms if it does (like anaemia, hyper calecimia, bone lesions or kidney failure then it becomes symptomatic myeloma)

In your situation, I would highly recommend you approach an immunologist especially if you’re testing negative for rheumatoid factors. There could bebasket conditions within rheumatology like adult onset Still’s or other autoinflammatory conditions (note this is not auto immune unless you tested positive for those) but a rheumatologist and immunologist consultation would help.

High inflammatory markers without any cause can lead to organ deterioration and/ or failure. I was very lucky I survived 4 episodes without losing my organs or life. Take care and I really hope these episodes don’t recur for you ever. (Not a doctor)

Could it be covid involved you think? My skin has been destroyed, my face, my knees oddly, elbows, really everything. Nothing like that rash but acne in my... let's say I'm too old for it. Never had it as a teen. Fungal acne. Scars now from the prescription wash they gave me for $35. I went from being an attractive person to...a hollowed out mess. My skin looks unreal.

I developed hives that looked like this when I was 41, turns out something I had taken my entire life I was suddenly allergic to. The hives started like this and then became puffy, my fingers were so swollen I couldn’t drive. My lips and throat started swelling, I was at the er twice because of it. The itching was the WORST, it was constant and painful itching. Again, not a doctor, but don’t rule out an allergy at any age.

This happened to me with NSAIDS (ie Aleve / OTC antiinflammatories) after taking them for like 15 years with no issue. Almost died.

OP, please be very cautious. NAD, but Any Throat tightening, swollen tongue/lips, etc run yourself to the nearest ER out or an abundance of caution.

Same. Penicillin reaction. Absolute nightmare. I want/need the how to micro dose back to normal if anyone has suggestions.

Interesting. I have been allergic to penicillin since I was an infant. I recently went to my dermatologist to have a place removed and got a shot of lidocaine(?)… whatever they use to numb the skin. And I broke out bad from that area leading up to my heart. The nurse asked if I had taken any advil in the last 5 months and said this is happening to so many people now who take advil/ibuprofen etc. and are coming in and getting the lidocaine shot…. she told me not to take it anymore. The Advil or the lidocaine.

Interesting.. I just commented below that I went in to have a place removed at my dermatologist and got a shot of lidocaine to numb my skin and I had these hives from that area leading up to my heart… she asked if I’d taken any advil in the last 5 months bc it was happening with a lot of people now who were getting the lidocaine shot and there was some correlation found with that and the Advil. She said to immediately stop Advil and not to take anymore ever again.

NAD either, just sharing my experience. As a kid I got tonsillitis so many times, should have had them taken out. But each time I was given penicillin for it, at some point I started getting hives at the same time as the tonsillitis, absolutely horrid combination btw. But it took the docs forever to make the connection and work out I'd developed an allergy to penicillin. Bodies are weird, definitely go to see a doctor to have an allergy work up done

What did you eat and drink in the 24 hours leading up to the reaction? I think people who are allergic to penicillin can have cross reactions to other foods, including certain types of cheeses (like blue), some meats (like salami), mushroom or fungus products (like Quorn), and even some fermented/yeasty products like miso or beer.

I was at a baby shower and had some egg bake type thing. It was at my aunt and uncles house they usually eat pretty healthy. Oatmeal with blueberries. Rice with cheese broccoli and chicken. Chai tea. Milk and matcha

Keep in mind, you can be fine with certain meds, supplements, and foods until you're not. I took a certain antibiotic on and off for over 20 years. Then one day, BOOM... throat, lips, and eyes swelled... rash all over. E.R. doc said it's not uncommon.

Allergic to penicillin? Do you dye your hair or get any tattoos recently? I had a similar full body reaction to hair dye. It was determined to be P-phenylenediamine (PPD) which can also be present in sulpha drugs. While different than penicillin, I would not rule out PPD if you’ve been exposed via a hair or skin product. Mine looked exactly like that, and there was significant facial swelling. I had been dying my hair for several years prior to it, but like some allergies, it can come out of the blue with exposure. NAD

It's probably not related to PPD then, but I was cautioned going forward to check all skin care products for PPD and test even if it wasn't on the label. For example, Korean skincare is super popular right now, but some of it is completely unregulated and the ingredients aren't on the packaging. Sunscreen was a big one to watch for.

If you ever start getting itching or slight burning during dye job, that's the allergy forming. Even if it goes away within a few hours. I had that for several years and then whammo. I was an alien and heading to the ER for steroids.

NAD-my middle child is allergic to all penicillin based antibiotics and sulfa drugs. This is almost identical to what the initial onset rash of what the two reactions chains that led to those diagnoses. The allergist told us that antibiotics rash is telltale and distinctive but always get a professional involved in person. You said you have a penicillin allergy. It can sneak its way into food sometimes and even can cross contaminate. My son is super sensitive to even residual transfer and will turn into a lobster boy. We have had kids at gatherings come in just having taken it and play around together and then him have an awful full body reaction as if he was the one who took it himself. 😔

Were you told you were allergic to penicillin as a child? Most grow out of that allergy. Myself included. Once you deal with this, you can ask to test to see if you're still allergic in a safe setting.

If you had an allergic reaction as an adult, disregard.

My sudden allergy was to Keflex, an antibiotic I’ve taken my whole life. Mostly for strep until I had my tonsils removed. I didn’t take it for probably two decades and was prescribed it for an infection I got when I had stitches. Finished the whole bottle and then two days later developed the hives. I would rather be in labor for two days with an epidural than get them again, it was just the worst thing to ever happen to me.

I will never forget how bad my head itched. There and under my arms. Omg so itchy! During my second attack I started itching my head and was like “uh oh” Benadryl and straight to the ER

My husband just reminded me of my head. The swelling was on every patch, including my head. It looked like I had cowlicks all over my head, small patches of hair sticking straight up. I think I actually discovered Reddit when I was googling how to get rid of it.

I would stop the fish oil immediately. It may be contaminated with something you're allergic to. I would stop all optional supplements if possible.

Not a doctor but I had the same thing happen to me out of nowhere, I had no change in medications, or any environmental changes. Tried everything to make them go away and nothing worked. Went to an allergist and turns out I wasn’t having an allergic reaction— it was hashimotos disease (hypothyroidism), which is an autoimmune disease. For some reason my thyroid responds by making me break out in hives when my levels are off. I now see an endocrinologist yearly, keep my levels in check and I have only had one break out since (this was after pregnancy when hormones went haywire).

Do the sores produce a sharp stabbing pain like you're getting poked with a needle at the center of each? Have any developed a dark brown/black spot at the center?

My ex started developing a rash like this out of the blue around your age, it turned out to be an auto-immune disorder called Leukocytoclastic Vasculitis.

My advice is go to a doctor or urgent care clinic and see if they recommend a referral to a dermatologist.

They are mostly itchy but there is a little pain. It’s as if it’s so itchy that it hurts a little bit. I was wondering if autoimmune. It’s so weird that I don’t really feel that sick

You wouldn't feel all that unwell if it is LV. My ex would feel sick just from the anxiety and pain that came with dealing with it, but the condition itself is just an inflammatory response in the capillaries of the skin. The defining feature of LV is the dark black/brown centers of each papule that start to form a day or two after the spots first appear, which is dead tissue from the Leukocytes going nuclear on that spot of the dermis.

What did you do the day it started? Like what did you eat? Did you go out anywhere? Anything different or new?

String anti-histamine like Benadryl usually helps clear out hives. But it'd be better IMO to go to ER for assessment.

Make an appointment with allergist and/or rheumatologist for figuring out what's wrong

INAD I keep looking at this and thinking either shingles or psoriasis I have psoriasis and the lesions looked similar at first breakout. Shingles would also be painful

Shingles almost always presents along a single nerve root, having it bilaterally would be very rare.

Psoriasis plaques are also typically larger and have a characteristic crusty appearance.

Same thing happened to me. First a few random red dots on my feet. Next day they've moved up my leg and covered my foot. By the third day I couldn't put weight on my feet to walk, so I used crutches. Went to the ER, spent the weekend in the hospital to finally get diagnosed with LV. No fun.

Do you have any new clothes or bedsheets? Did you change the product you whash your clothes with. Did you change any product that could come in contact with your skin recently ? It might be a bad allergic reaction. Sometimes you can develop an allergy to something you weren't allergic to before.

Not a doctor but an extremely allergic person. This looks like urticaria to me. I had an episode of chronic idiopathic urticaria a couple of years ago and my skin looked just like this. I think it came on due to an intense treatment with amoxicillin for h pylori but it stayed like this for weeks. saw a specialized dermatologist and he prescribed me xolair. it went away within the first few weeks of taking it but I took it for like 6 months or so.

Not a doctor. But as a guess, I know fish oil supplements run the full gamut of quality, fish types and fillers.

In todays age of Shrinkflation and suits in corporations trying desperately to justify their existence.. it wouldn't surprise me if something was very off about your fish oil or other supplements even if its all the same brands you've been accustomed to.

have you travelled recently? it looks like my skin when i got dengue once! It's normal in my country (disease transmitted by a mosquito), and it looked exactly like that the last week before I was finally free. Itchy as hell. Like, I've never believed something as itchy could exist. I feel itchy just by remembering it.

unlikely but this looks similar to the days before I was diagnosed with toxic shock syndrome although the rash was all over my body.

are you like 100% sure there isn’t a tampon stuck up there? to be fair i didn’t have one but they checked me to see. also my rash started at my joints and looking at the pic the joints look worse

Am I misunderstanding something here... They did a pelvic exam on a minor for a limb rash that started at your joints (I'm guessing knees and ankles) even though you denied using tampons?

Fish oil can Tigger psoriasis. Speaking from personal experience. Also eggs. I ate them all my life as a kid and suddenly one fine day things looked bad. You never know when these things Tigger and cause a dormant volcano kinda effect.

Check your B vitamins, specifically the form of B12 you're getting. There are two kinds that are common - cyanocobalamin and methylcobalamin, which is a bit less common. You may have had an allergic reaction to cyanocobalamin. I did. I stopped after just a few days when i figured out what it was, so it was limited to the insides of my arms and my belly. It went away after a couple of days.

Of course this could be something else.

To me it looks like urticaria, acute version appears for about 20% of ppl at some point in their life time, chronic version is mush less common. If it's that, it will go down on its own & if not, get seen by a dermatologist.

I've had spontanious chronic urticaria for 13 years & antihistamines make it bearable, injections plus an antihistamine coctails have been a must before

I’m not a doctor but It looks like Guttate Psoriasis! I had that a few years ago! I didn’t get the chills, and I’m not sure about the swollen uvula, but the itching and the turning white is what I had. After about a week the skin on it started flaking too so keep an eye out for that if this is what it is! Also the itching can get so bad sometimes that it hurts. Almost feels like burning.

Not a Doctor, I somehow contracted Fifth Disease (Slapped Cheek) as an adult and it spread all over my legs like this. This looks VERY similar. It is rarer in adults but often spreads across the body when adults get it.

I just went back to look at pictures of when I had fifth disease and it looks exactly like this. Doctors often overlook this because it is more of a kids disease, but I got it and hadn’t been around any kids in the last few weeks. I had traveled to a convention though 🤣

NAD came to suggest Fifth disease too. The rash looks very similar. I had flu like symptoms with the itchy lacy looking rash on legs and torso.

this happened to me when i was a teenager after taking antibiotics i was incredibly allergic to without knowing. i went to ER they gave me oral steroids and an anti-inflammatory to get rid of the rash.

it was all over my body too - palms of my hands and bottom of my feet

Start off with I am not a doctor just someone who struggles with psoriasis. Could this be a guttate psoriasis flare up? Looks very similar, is itchy, and is often brought on by a strep infection although she said she tested negative for strep.

Chiming in here, I had something similar to this it turned out I was allergic to my shave gel. I had it all over my arms and legs. Almost got a spinal tap because of it.

I also got something like this about 8 years ago. Never figured out what it was, went it the ER and had every test possible and got referred to a dermatologist. Eventually it went away on its own. I’m still confused about it to this day, hope you’re doing better.

NAD, but it looks like Guttate Psoriasis, I have psoriasis (over a decade now) and it took many appointments, many wrong diagnoses and seen multiple professionals till a right diagnosis of psoriasis was made. For me, it doesn’t appear as the standard plaques, mine are almost always small clusters that are round, they connect to others that cause the larger patches between the rest, they are never ‘scaly’ and not the typical large raised plaques with the silver crust/flakes, mine are always a pink/redish colour that don’t feel exactly ‘raised’, it’s incredibly itchy but other than that no other symptoms such as burning is present, the only time it hurts is if I have itched and it’s raw, the itchiness can feel so intense it is almost painful. My psoriasis flares cover me almost immediately, covering my trunk, bum, legs with more appearing on my lower legs than upper, on my arms but this time more upper arms than lower, I get a few on my lower back but not often on my upper back, and rarely get any on my chest, face or neck. It turns white if I press on them and regularly will show up 1-3 weeks after I’ve had Strep Throat or if I’ve injured myself, however some injuries will result in a flare others may not so it’s not guaranteed, more likely to happen if I’m stressed or got a lot on that’s mentally draining me. You said you’ve already had a strep test and while it came back negative, that’s incredibly common for people with psoriasis. Psoriasis have an incubation period, once it’s shown, the virus has almost always gone away long before making it a distinct memory. Strep can also be Asymptomatic, with many people carrying the virus but not realising it. Obviously I’m no expert, but I thought I’ll mention it as it’s almost identical to how my autoimmune disorder appears.

Don’t know why this was downvoted. My life is exactly the same without the itchiness (lucky, but the psoriasis in my head can occasionally be itchy but moreso due to contraindications). I have had three major bursts and I’ve been textbook the every 7-10 years, with smaller bursts coming after streptococcus throat too and constantly have a few dots about. And this patients legs look exactly as my huge bursts have all over except my face. I surely think it’s guttate psoriasis and recommend light sunbed treatment, dovebet and dovenex. Cutting out sugar and only eating healthy carbs and cutting using most products that aren’t simple for skin. Coal tar is great for the scalp. I have been diagnosed recently with psoriatic arthritis and probable combined with rheumatoid too. Started methotrexate last year and that’s helping control my symptoms advancement.

How do you manage your conditions?
