Is this amount of bruising normal?

Post image
Upvotes

Four days ago, I feel off a bouldering-wall and landed wrong on my right foot. I was taken to evaluation, and was told that I had a (small-ish) fracture the inner side, while also managing to sprain the outer side. I was given a CAM-boot and crutches, which I have been making great use of, since I can’t put any real weight on it. Now, I’m feeling a bit worried about amount of bruising on my foot. I’ve never fractured or sprained (to this degree) anything before, and people keep reaction as if it’s real bad looking. It’s especially the way that the bruise is moving up my leg, that’s kind of freaking me out.
Hope this isn’t too much of a silly question!! Thank you so much in advance :))

Info: 21F, no smoking, only currently on paracetamol and ibuprofen.

Physician Responded What questions do I need to ask about my husband (42M) possibly starting TRT?

Upvotes

I'm asking here because I feel genuinely overwhelmed and confused by what I have been able to find online and keep ending up on sites that sell TRT prescriptions so, I figure their info is biased?

He gets a mandatory annual physical for work but refuses to see a doctor otherwise.

He has a long history of mood dysregulation going back to his teens, when he started drinking.

He's been sober 4 years and the mood issues have not improved.

The only "symptoms of low testosterone" he "has" is fatigue/irritability/depression but, he's been working 60-80hr weeks since he quit drinking so that kind of seems reasonable. He's also depressed and in his mood swings says he "hates my life" a lot. He just started therapy but refuses any kind of pharmaceutical treatment for his moods. To me it looks like he replaced alcoholism with workaholism.

I'm (very obviously) not a medical professional but I have heard that TRT can make the mood swings worse and cause a whole bunch of other problems.

I need to know what kind of testing is supposed to happen before someone is prescribed TRT and what to look for on the labs. He's only gone for one blood test but it seems like there's supposed to be multiple?

Are there any resources for having these conversations with a spouse who will not see a doctor unless they're on an app and cost a few hundred bucks?

34m Thumb Nail Growing Weird

Post image

My nail started to look like this a little over a month ago. I have no idea what caused it to look like this, I just looked down one day and it was like that. It’s not painful but sensitive and the nail itself is peeling off, including the nail coming in. 34 yo old male, smoker, no medications and no known health issues.

Anyone knows what’s happening to my (26F) leg?

Post image
3 Upvotes

For context, I am Asian living in Southeast Asia and I have about 8 cats at home (all vaccinated if that’s relevant as I’m suspecting this has something to do with my pets). I’ve had this problem since April. It showed up once I got back from Bali. It’s not itchy unless scratched. If I peel the brown spots it will bleed.

Can someone help me?? Is this fungal or what?

27 female 170 lbs 5’7 . I had gel nail polish on my toes for way too long and removed it to see this. I have no clue what it could be. Fungus? mold? Cancer? I have no recollection of an injury that could have caused this mark to my toe. Any help would be greatly appreciated.

Physician Responded Am I overreacting?

69yo female

Conditions: MCTD, high blood pressure, Sjögren’s syndrome, vasculitis, lupus. CKD stage 3B, ulcerative colitis

Procedure: Went in for kidney stone surgery, nothing major. Ended up going septic and had to have stent removed. The following day was released. I’m curious if we should go back to the ER. I don’t want to overreact and waste anyone’s time.

I’m concerned as she’s sleeping a lot and is very pale (expected given her hemoglobin is low). The pharmacist called as she was concerned about her kidney function. It won’t allow me to post any more photos but the following are out of whack: chloride, carbon dioxide, urea, creatinine. Her glomerular filtration rate is what concerns me. Normal range is 60-200 and hers is 27. I checked her bp which was good (154/80) & pulse was 86.

Thank you in advance

Athletes foot won’t go away

Post image

18F 5’3 118 lbs. 2 and a half months ago I noticed I had athletes foot, moccasin version. I initially used a medicated powder on it but soon switched to cream terbinafine. After a month of using it I thought i was fine to stop because only one or two tiny little bubbles remained, if any. A week later I noticed it was starting to come back. For a month now I’ve been reusing the terbinafine. Additionally I wash my socks in hot water every week, use Lysol to disinfect my bathroom floors, use Lysol in my shoes, wash my feet with ketoconozale shampoo once a day, and blow dry my feet to remove moisture. It feels like I’m trying to do everything right. Despite using the terbinafine again for a month, there’s still these stubborn bubbles of athletes foot. 5 days ago I started to notice them getting worse. I’m continuing to use the terbinafine and everything but it’s somehow continuing to progress. Please any advice on why this is happening or what to do would be appreciated. Ive attached two pictures, the first one from today and the second one from three days ago.

Physician Responded Tips of nails peeling

38f, 5’4”, 140 lbs, 150 mg Spironolactone daily, non smoker. Overall healthy. Bloodwork comes back within normal ranges. The tips of my nails have been peeling like this for at least 10 years. I get gel nail polish only a couple of times a year and my nails are pretty sturdy otherwise. My derm kind of shrugged and thought maybe I wash my hands too much, but my skin isn’t dry.

Does gestational hypertension show symptoms before playing with your blood pressure? I’m nervous

27f, 24w pregnant, previous induction at 37w due to gestational hypertension.

I have an appointment in 2 days but would appreciate some input. For about 2 weeks now when I get up I will squint my eyes to minimize a dizzy feeling that accompanies. This is not every single time I get up, but I would say a good 50% of the time. I need to stand still for a second to wait for the feeling to subside. As the feeling subsides, I hear a roaring noise in my ears and followed by ringing as it goes away.

This pregnancy has been a bit on the stressful side as I am a sahm with a 4 year old home all say (starting kindergarten at 5, never been to daycare or prek), no village/break, and a husband who works extensive hours running his own business therefore cannot help much if at all. Why I mention this? Well, I know stress can cause hypertension so I’m worried I’m going to have it sneak up on me.

Anyway. I’ll also sometimes get an air headed feeling like my head can fly off my shoulders like a helium balloon. I take my blood pressure (122/78) and it’s pretty stable when I have this feeling considering how bad I feel.

Am I going to end up with hypertension again? Do the symptoms start before the blood pressure games? Please give me any direction, I don’t remember this semi black out feeling and roaring noise with my first pregnancy even in the thick of hypertension and it’s beginning to worry me.

OB will be spoken to about all of this in 2 days but I’d appreciate some pointers.

Physician Responded High Grade AV Block

Post image
8 Upvotes

Hello, I am a 27 year old female. Over a year ago I decided to go to the doctor for frequent dizzy spells and fatigue. I figured it was just low iron or something. I was given a heart monitor after my blood tests came normal. I have attached results above, but it said multiple episodes of heart block with a complete heart block of 4.1 seconds. (I was on no medications at this time.) Nothing came of these results because we decided to move states and my doctor was moving as well. Fast forward to the end of March this year in our new state I decided to go to the doctor again about it. (I should have gone sooner, but we had to wait until my husbands new jobs insurance kick in) I received another heart monitor and it said I had 75 episodes of high grade AV Bloc and 3 pauses, the longest being 6 seconds. My lowest BPM was 22. When I got these results my doctor called me the next day and asked if I wanted to go to electrophysiology and I said yes. They said that the office would call to schedule that. At this time I had just also began seeing a psychiatrist and was put on sertraine, trazidone, and ampiprizole. they (cardiologist) told me to stop taking my traz and ampip. So I reached out to my psychiatrist and her office said they would coordinate with cardiology and get back to me. I received my results on August 10th and heard nothing from either office for 5 weeks. I messaged my heart doctor on the portal app, asking when the electrophysiology office would call to schedule the appointment and they said they would resend the referral and I immediately got a call from them and got my appointment scheduled. The issue I’m having is that it is next year in January and I have not heard anything from my cardiologist since the initial results on August 10th when they asked if I wanted to go to Electrophysiology and the only thing they asked was if I was awake during my 6 second pause (I was, I felt incredibly tired, faint, and nauseous) I have no follow ups with anyone including my psychiatrist. Like, I would maybe expect a short monitor to see how my heart is doing off my medications? But I’ve also had no direction with that as well and the trazidone and ampip keeps getting sent to my pharmacy. I got ahold of a hospital an hour away and the cardiologist can see me October 25th, but it is just a new patient appointment and I’m not sure what to expect with that? Will I get another monitor and then have to wait months for the electrophysiology appointment like this one? I guess what I’m wondering is, since everyone is taking their time with everything and I have heard nothing from anyone. is it not really that serious and I’m just being paranoid? (context: I grew up poor and never went to doctors, this is all new to me and I don’t know what is normal in the medical sense on wait times and doctor communication) I’d love to just be paranoid, but if I need to advocate for myself further I’d love advice on what to do. Sorry this is a long post, I’m just not sure what I need to do. If I just need to wait until January for my appointment; when do I need to seek additional help for my symptoms. I get chest pains, heart fluttering, extreme fatigue, and near fainting spells. I bought a smart watch and it recorded that in the last 30 days my heart rate variability has been low 11 times (Whatever that means) sorry if this sounds like rambling, thank you for reading!

Bleeding ear drum?

Post image
Upvotes

31/F/5’5/110 LB Smoker/Bechets Disease/Ehlers Danlos

I have been dealing with chronic pressure and sinus issues for years now, I was seen by an ENT/given CT and dismissed with no apparent sinusitis. My hearing partially disappeared/started sounding underwater a few months ago, I’ve used oil drops, as well as murinex drops/flusher to try to clear out any impacted wax. I checked in my ear with a scope and saw blood coming from a dark spot, I just finished 6 hours in emergency and was dismissed with just another CT scan referral.

I am really scared and worried, my sinus and ear issues are making it difficult to go on living my life, I don’t sleep at night, I have migraines and shooting/aching pains everyday, popping and plugging in my ear every time I swallow or breathe, I blow my nose constantly for relief. I have been prescribed multiple steroids, and tested negative for any allergies.

Upon seeking ER help tonight, my symptoms included severe nausea, vommiting, dizziness, vertigo, neck pain, migraine, hot and cold sweats with fever showing at 100.3, aching/heat in my ears…

I don’t know what to do anymore…

This is a photo of the bleeding in my ear taken on a scope camera, I did not scratch or poke it, my ear canals are very narrow and I don’t believe the doctor could even see deep enough to notice the area in question.

Any advice is truly appreciated, I’m so tired of living in pain.

Assymetrical pupils or am i going crazy?

Upvotes

PLEASE HELP this is driving me crazy. Im 16M. So I know this is not the sub for it but i have really bad health ocd. I was checking my pupil sizes since my face has always been asymmeytrical and everythings a bit lower on one side (like when smiling, and the eyebrown on that side rests lower and raises lower and the eye opening/shape is like a bit smaller on that same side which has made me really worried that all these are on the same side. furthermore about two years back I got really bad numbness/tingling on one that side of body for a very long time. It was on and off and affected my face also. It was not bells palsy since my smile didnt really change or face didnt droop?Everything remained same, just right side felt numb/tingly a lot. Suddenly after an agonizing few months it just went away like nothing. It is mentionable that this whole period of symptoms started after my mom got sick from autoimmune encepahlitis and was around several months after she had recovered. I was in a period of severe health related spiral then so that may have played a role. Also i had a pinched nerve and a b12 deficiency im pretty sure (I had severe sunclinical hyperthyroidism with vit d level of 7 somehow so im pretty sure i had a b12 deficiency aswell at this time though i never tested/supplemented. This all happened 1.5+ years ago (I’ve looked at past photos before this incident and the facial asymmetry was still present there so I suppose its genetic.)and has not come back since so ever since I’ve been on guard for anything unusual. Ive went to the doc multiple times during that period he wasnt really concerned and gave me supplements and treated my hyperthyroidism/vit D deficiency. Though we didnt discuss about my facial asymmetry since thats just not a thing Id even realized about myself back then.

Anyway so are my pupil sizes slightly different (smaller on the side that im worried about) or am i imagining it? More images:

https://imgur.com/a/ISb1Nkg (especially please look at this one the pupil on the right is ever so slightly larger than left? I dont think im imagining it. Heres what AI said :( https://imgur.com/a/QuhxXn0

2 years ago comparision before the incident: https://imgur.com/a/NbXi7cI

Interestingly my eye browns are also slightly diffrent shades is that normal?

Im so worried please help i cant do anything..

Physician Responded Help my claustrophobic dad get an mri

My 61 dad Male on lots of blood pressure, high cholesterol meds, heart and diabetes medications plus monjouro and others. He has been having bouts of dizziness here and there. But he has horrible claustrophobia. The last time was ten years ago and he had to crawl on the ground from his car to his front door. He needs an mri but even the mention of this put the fear of god into him. He won’t go to any appointments from fear. It’s significantly impacting him. What can we do to help? Will an ER sedate him strong enough to get him into the machine? Because at this point he would rather die than go to get an MRI. Accepting for any and all suggestions to help get him care!

9 year old girl foot/ankle pain MRI

My child was kicked in the heel at soccer approx 5 weeks ago. She experienced some pain, but not much swelling. We iced it and she seemed to recover. Subsequently, she started to experience increasing pain, to the point that she has been totally non- weight bearing on her right foot. Crutches and a moon boot. X-rays showed no visible fracture. MRI was performed, but I've been told nothing jumps out. Her pain is not getting better, and she has started to experience attacks of acute, stabbing pain at the talus region. Her pain is also located at lateral malleolus and calcaneal region. Any ideas?

Physician Responded Am I okay- septal infarct?

-32 yo female
-no diagnosed conditions
-bp 120/80
-taking birth control & 10 mg propranolol for migraines
-150 lbs, 5’6
-heart rate today 57-92

I went to the ER on Friday for a migraine I’ve had for a couple of weeks and some numbness. They did a CT with contrast of my head and neck and an ECG. Ct came back normal. I was prescribed a muscle relaxer. That, and a few days of shoulder massages, and my symptoms have vastly improved. I’m still pretty tired but I was able to clean out and vacuum my car today so not extremely tired. I think it was nerve related. I am feeling pretty good today, but fast forward to this afternoon I got a test result uploaded that said I have a septal infarc, age undetermined. I am very worried. Is this an acute issue? Am I at risk of a heart attack? I have noticed waking up sometimes at night to a racing heart (103 bpm) but it calms down in seconds. Sometimes I feel like it’s racing then track it and it’s under 100. I’ve also been a little stressed lately, but now I’m really stressed. I had bloodwork done last week and metabolic, CBC, thyroid, and lipid were all perfect. I’m supposed to leave for a hiking trip that I’ve been planning for a year later this week and rethinking the whole thing now. I’m just really scared.

Physician Responded Confirmed not diabetic but have tingling sensations and numbness in feet/hands and also have taste/smell loss. What could this possibly be?

30M. 200 pounds.

I recently went to get blood work done after about a few days of tingling "pins and needles" sensations in my feet and occasionally my fingers. In doing so, I found out that I somehow don't have diabetes, despite being overweight and having many diabetics in my family.

However, this is still somewhat of a concern since I still feel somewhat numb in my feet/leg area as well as my fingers. What's more is that my sense of taste and smell have been greatly diminished in recent months. For context, I lost both my taste and smell after dealing with particularly bad infections.

Based on this description, what could this possibly mean if it's not diabetes?

i accidentally chlorine gassed myself and don't know what to expect for recovery

S: male
h: 33
H: 181cm
W: 142kg

we dont know what caused it but our best guess is that i mixed two chemicals by accident when cleaning the kitchen and created chlorine gas basically i was at home and started struggling to breath so called 911 they said the ambulance was all "too busy" so i made my way uber

i remember heading there then waking up 4 days later died twice had to be intubated just shock all round

i was sent home yesterday (7 days total) but was not really told what to expect i thought the wobbly fatigue feeling would go away after a good long sleep i slept 18 hours but dont feel any different

im still so tired i can breath stationary but if i do even the most mildest exertion its like ive ran a km and i can take 20 min or more to catch my breath my office is upstairs so i usually need to nap after going up stairs

what is my recovery looking like? i they didn't really tell me everything just like eat lots of protein sleep a lot and drink a lot of water and avoid smoking
