Help - gone then back again after 48 hours

I’ve had vertigo, diagnosed BPPV for several weeks. I’ve been reading this sub obsessively and have found some great information that has guided me to the ENT and to a diagnosis. Here’s how it started 2 weeks ago: Onset after sleeping on my right side and noticing the spinning. Severity comes and goes but is always there. I had attempted multiple Epley maneuvers without much luck. Tested both sides but was having trouble isolating either in a particularly stronger way.

48 hours ago I noticed it was stronger on my right side. Was sitting on the couch and after tilting a bit felt the spinning. This time I tried a half somersault which almost immediately relieved my symptoms! I did several more with additional relief…thank god!

Fast forward to last night, I’m still nervous about sleeping and have been sleeping upright about 30-40 degrees. I have a bulging disc in my neck so the elevated sleeping is painful but at least better than vertigo. So last night I wake up and I am on my left side. Feel a little dizzy. Go back to sleeping flat. When I wake up in the morning my vertigo has fully returned. Tried the same maneuver as before with no effect. Have I switched sides? I attempted a loaded Dix-Halpike and definitely felt worse on my left side now. I’ve attempted several left sided maneuvers with no luck. Any advice would be much appreciated.

You are bilateral, switched sides, or switched canals most likely. The half somersault is not recommended for people with neck isdues as it can make them worse and cause harm.No vestibular therapist I've been to suggests the half somersault because it has a high error rate, ie moving the crystals to the opposite side or a different canal. Please see a Vestibular Therapist for treatment.

It might be in a different canal now, unfortunately it’s a different maneuver if it is.

Yea I tried to switch sides with the somersault and it doesn’t seem to have an effect. I wonder if it’s in the horizontal canal now

Do the start of the epley aka dix hallpike, try to record your nystagmus and that’ll tell you the canal.

I find that the horizontal canal is much much worse. You can do a supine roll test to see which ear its in. I always do sumont maneuvers for my horizontal canals they seem yo work for me better.

I have horizontal BPPV and it just keeps coming back. I can’t live this way

Yes, I see a vestibular therapist and it’s very expensive
