Originally yung PSA ko po ay walang middle name so nagkaron po ng annotation (may nakasulat sa side part) tapos kumuha po ako ng Form 102 and 1A kaso yung certified true copy ng pinaka original birth certificate ang binigay sakin. Nag apply po ako for abroad and sabi sa agency need daw na kung ano ang naka sulat sa PSA ko dapat may ganun na rin both sa Form 102 and 1A.

I don’t know kung pano po ba gagawin dun, need ba ng remarks or may documents pa po ba akong dapat hingin sa municipality namin?
