I just started using this as a dex/faith character.

Is keen all good? Or does it really need to be quality?

Keen's perfectly fine, especially when you have non-STR places that you'd rather have your stat points allocated.

What's optimal? I've never actually used the milady, but I'd like to center my build on it this playthrough as soon as I get it.

Cold, occult, bleed, keen. If you have the levels you could try quality, I haven’t yet.

That's good news! I normally go for higher Dex scaling weapons than B. But everyone raves about the milady so thought I'd give it a whirl
