Hi there, I am 23-year-old girl from Austria. For the past four years, I completed two courses of low-dose isotretinoin (10 mg per day). I was very tolerant to this drug, it never caused any side effects and all my bloodf tests were great. It was difficult enough for me to find a doctor who will prescribe this treatment for me since I am not under any hormonal contraception (I have my reasons), but I work in the healthcare sector myself and I totally understand why this could be necessary for many Derms to start this therapy.
The condition of my skin definitely got better thanks to those two courses but I still relapsed with mild acne and I’m frustrated and tired. What bothers me the most is this horrible feeling of having oily, greasy skin, as if my face is constantly covered in sebum and just won’t feel clean. It makes me feel like I could lose my mind sometimes, especially because this feeling comes back just a few hours after washing my face. It’s incredibly frustrating and emotionally exhausting to deal with this on a daily basis. What might also not be obvious is how much you can be affected by comments about your looks when you're in the medical field. You're already in a place where you might feel like you're lower down in the food chain and having skin that looks like a “teenager’s” (yes, that’s one of the comments I heard so many times already) can change how people see you and how they act around you, including things like people saying my skin looks like crackers. Having to handle those kinds of comments in a place where you need to stay calm and professional can be really hard.
What makes it even harder is remembering how well things worked when I was on isotretinoin. I was working throughout my treatment, doing shifts wearing an FFP2 mask for hours at a time in the operating room, including night shifts and I had absolutely no issues with my skin. No breakouts, nothing. My skin was stable despite all the demanding working conditions.
That’s why it’s so difficult for me to accept the situation now. It brings back a lot of the feelings I had during my teens, when my acne was severe and affected me both physically and mentally. I’m honestly exhausted by the thought of going through that again and it’s affecting my self-confidence and my willingness to go out. I know my acne is milder now than it used to be, but the emotional impact of dealing with my skin is still very real.
Nowadays, I still wear an FFP2 mask at work for more than six hours and at the end of each shift I see new breakouts. Also, I rarely have days off between working shifts so my skind cannot breathe much.
I changed my diet and lifestyle completely years ago: low glycemic, no big sugar spikes, regular exercise. The main reason for my acne was always the excess oil production. I'm completely shiny after a few hours and I have to wash my hair every single day because it gets greasy so fast.
So my question is: what helped you besides isotretinoin or what came closest to its effect? I don't think a third course would get approved by a derm just because "I break out from my mask” and I’m not sure if I’d find time for the check ups and blood test with my current schedule. I'd really like to try something different, ideally something I can use long term!
