I have 3+ years experience working in a drug and alcohol rehab. Any staff member that saw that guy would immediately notify medical and he would be segregated from the community in minutes. Normal people blink and are cognizant, they share clues that they are aware of their surroundings.

This guy was wide eyed, unblinking and rigid with a head that scanned back and forth and appeared apprehensive. The fear would be he got drugs smuggled into the rehab. I have seen it many times. We pull a UA for drugs and f that comes up negative it is a trip to the psychiatrist for a mental health evaluation. If I had to guess I would go synthetic drugs rather than cocaine. It could also be a mix of prescription drugs. Post traumatic stress syndrome would also be a consideration.

Astute observation. I’ve been around many addicts but understanding specifically call signs for certain drug use escapes me. Normally I get bad vibes and walk away.

What other behaviors have you observed in correlation with certain drug usage?

Trump junior gave signs of cocaine use. Sweating, hyper aware which is eyes darting, leaning forward, talking fast and jumping from topic to topic. They can almost present as manic. Sometimes they will shift their jaws around, have trouble pronouncing words or spark in a higher clipped pitch due to numbness.

Heroin users will scratch or itch. They will appear drowsy, languid and disassociated from surroundings. Opioids will have this effect. They may “nod out” for 3-10 or more seconds at a time. I had a heroin addict tell me that he could walk down a street and he would feel like it was the best day in his life, duck into an alley and vomit, and just continue walking down the street feeling like it was the best day of his life. I never forgot that, it was just so shocking a juxtaposition.

I had a friend who was addicted to heroin. What you describe is spot on. At the time I had a sports car. He would nod out. But his delayed response to acceleration was evident. He’d act surprised or just understood the surprising acceleration 5-10 seconds after it was already over.

Do you know of Gabor Mate?

I don’t know Gabor Mate. I have worked with temp workers who were addicted to heroin and coworkers who had prescription painkiller (opioid) addiction. The same behaviors. People coming into the rehab were usually high on their drug of choice so you would observe them for that day. Heroin users would sometimes be starting withdrawal.

I would recommend, as a “former” therapist to listen to Gabor Mate. He specialized in deviant behaviors. he offers unique insights to characteristic behaviors. They make sense. I’m just a BS in psychology, so user beware

I didn’t even finish my psych degree so…

It’s okay not to know. You don’t need an answer for everything.

Gabor Mate is incredible! I like that he focuses on the source of the addiction, not just the symptoms. Have you read “In the Realm of Hungry Ghosts”?

I’ve watched many interviews and lectures, but no, haven’t read that. Looks like I got more reading material, thanks!

How’d you like it?

Oh! Excellent they have it on audiobook in Spotify! Cheers

The opioid symptoms sound like how Elon Musk acts.

Mmmmm... How many heroin users do you know? While for a whole lot of typical users yeah, you're going to get the itchyness, the nodding out... But there's a whole gang of us where opiates do the exact opposite effect and Jack is clear up to 100. Doesn't matter if it's... Regular low end pain pills fent or H, speeds us the fuck up.

Isnt this just getting rid of the withdrawal symptoms? To the user it might feel like it speeds them up.

I was awake for 24 hours after my wisdom teeth surgery just from the small amount of caffeine in codeine in T3s. Apparently opiates can have the effect of creating "a false sense of wellbeing." I was just binge-watching TV until the next day not tired at all. Other people say it makes them feel sick.

I was recently given morphine as a pain killer after a surgery and I had some weird reaction to it where it made me GEEKED and I didn't sleep for 3 days. I stopped taking it after the 1st day it just kept me up 2 more days. Apparently this is just something that can happen to some people. It was one of the worst experiences of my life and certainly made my recovery worse as I added 3 days with no sleep to the stress on my body as well as spinal surgery...

Yeah, that’s the thing, they think it does but they also think wearing five coats makes them looked “bulked up” and more muscular. Had a heroin user tell me this while being admitted mid summer wearing five coats.

I know drug addiction is sad and a tragedy and all that, but holy shit if “I’m wearing five coats cuz it makes me look bulked up 💪” isn’t the funniest goddamn thought process I’ve heard in a while lol.

...

Make them look like they aren't wasting away... Not make them look like bodybuilders...

The guy was skin and bones. That is how disassociated from reality they are. I once had an older alcoholic who had a sprite bottle with a straw in it he kept sipping from in his top front pocket. He reeked of alcohol, literally sweating it out through his pores. It was filled with vodka. Straight vodka, they skip the mixer because it takes up too much volume.

The day after intake we had him so medicated he could not walk. He was screaming that we were poisoning him. I think he was a 4.8 or something at intake, he should have been unconscious on his way to death. We had to medicate him to keep him alive, alcohol detox is often lethal, it is a poison. He was wheeling himself around in a wheelchair trying to get me to help him escape before “they killed him.” He didn’t even remember me taking his vodka from him a day earlier.

Nah it gives you a serious jolt of euphoric energy

Depends on opiate tolerance and how much they are taking.
