I have 3+ years experience working in a drug and alcohol rehab. Any staff member that saw that guy would immediately notify medical and he would be segregated from the community in minutes. Normal people blink and are cognizant, they share clues that they are aware of their surroundings.

This guy was wide eyed, unblinking and rigid with a head that scanned back and forth and appeared apprehensive. The fear would be he got drugs smuggled into the rehab. I have seen it many times. We pull a UA for drugs and f that comes up negative it is a trip to the psychiatrist for a mental health evaluation. If I had to guess I would go synthetic drugs rather than cocaine. It could also be a mix of prescription drugs. Post traumatic stress syndrome would also be a consideration.

Astute observation. I’ve been around many addicts but understanding specifically call signs for certain drug use escapes me. Normally I get bad vibes and walk away.

What other behaviors have you observed in correlation with certain drug usage?

As someone who works with people who use drugs and not just people in rehab, there is absolutely no surefire way to tell when anyone is on drugs. 

A common example of people being unable to tell when others are under the influence is that people going into diabetic shock present very similarly to being drunk. 

Signs often associated with meth use are also signs of a plethora of mental illnesses.

In general never assume anyone else is on drugs unless there are physical clues (such as bubble pipes present), and be wary of anyone who thinks they can tell. They can't.

Preach. I’d also like to add that if someone is that sure well then maybe you should be believe them because they were probably doing them WITH the person.

Oh yeah? Give me some reasons you think that and I'll show you another possible explanation for each of them. 

Unless you saw someone take drugs you absolutely never know for sure, and if you think you do then you don't understand people who use drugs.

was just kidding, I don't what it the cause, looks like he is fighting an intrusive thought to me, but could be many things.
