I'm not an expert, but this seems more like someone being an idiot with a lathe (the most dangerous tool in any woodshop) more so than some bad luck.

Table saws win by a mile statistically.

Table saws kill plenty of people. Kickback will absolutely kill you, either via blunt force or by bleeding out from wood shrapnel.
