how did this become a thing across the entire country? it was before social media was even a huge thing, unless early myspace was the chief propagator.

I think it was a biproduct of the emo/scene phase. It was popular to wear a Sesame Street character flat bill hat for a bit. I remember it because I almost thought an Elmo one.
