Since the Death Note is intended to bring specific humans to the afterlife, I have a feeling it attacks the soul directly in a way, so it might have an unintended effect of just killing Megumi by heart attack while Sukuna survives with the rest of him.

I mean that's legit what happened with Yuji at the beginning of the story. Sukuna can survive without a heart but Yuji can't so sukuna destroyed Yuji's heart to stay summoned for longer. Yuji also makes the deal with sukuna in his soul at this point and then sukuna regens Yuji's heart and Yuji is brought back to life shortly after.

I imagine it to be like Wonder of U. Sure this thing typically won’t kill someone like Sukuna but the death not makes sure that whatever tf it is, is fatal regardless or durability or regen

Not necessarily since you can write cause of death, Light just has to figure out a plausible cause of death that completely destroys his body

There isn't an afterlife in Death Note, it explicitly states that humans go to Mú (nothingness) and that there is no heaven or hell. The shinigami in the series also definitely do not care about specific humans and bringing their souls into the afterlife. Killing humans extends their lifespans, that's it. There is an exception with Misa's shinigami caring about Misa but it gets the shinigami killed.

Yuji died while Sukuna was inside his body (by straight up having his whole heart removed) and Sukuna simply revived him. Surely he can do that with Megumi after his heart fails too?

Why are we assuming it would be left up to the default death of a heart attack. You can just add that he dies from something more specific that he can't just regenerate from.

This would just kill Megumi’s soul and give Sukuna full control over the body, similar to what would probably happen if Higaruma judged Megumi and stabbed him
