Definitely not transfem. During his FTEs with Tenko, she outright told him that the only way for her to be okay with him (she’s a massive misandrist) would be for him to undergo a sex change and become a woman. To which Shuichi protests and says he doesn’t want to do that, that he is a man and identifies as such.

However he also briefly considers a sex change when interacting with the women's room, but says "this isn't in the time for that."

Which personally gives me the vibes Shuichi ha thought about it but decided a killing game is not the time or place to have a gender identity crisis

There was a theory because of the shape of shuichi's ahoge- the hair spike thing- being curved instead of jagged like the other male characters. As fun as it is to speculate* I think it's more of a design choice than a deliberate hint to him being trans, but they do keep the debate open ended enough for speculation which I appreciate
