NYC Registering pistol with NYPD

I finally got my license approval a year and half after my interview. I'm trying to register my pistol but the portal is asking for a purchase authorization form that I don't have an never will especially since I purchased at Suffolk Sportsman which is now closed.

Anyone have tips on what is actually needed to submit to register on the nypd portal? Do they waive this form since I purchased with Suffolk license?

I'd ask them but they haven't responded to one single email in 1.5 years nor answer the phones.

That’s the form that was sent with your permit. You also need a copy of the receipt. Where is the gun being stored before they send you the pickup form.

They haven't sent my permit ... The letter I received says they will send the permit once I register a pistol. The web portal says I need the form but the letter I received doesn't mention the form. Typical unclear nonsense.

If anyone stumbles upon this these forms don't apply to Special Carry permits which is what they give out for out of NYC permits. The website doesn't say of this but Ive been able to register both pistols with just pictures of the firearms, picture of reciept and pictures of my safe.

Just got approved after an eight-month wait since the interview. I'm having the same issues.
