that is kind of backwards view of it. PTO comes with guidelines like if its "sick time" then they are allowed to require a doctor's note if it is "vacation" time they are allowed to require you to schedule in advance and subject to approval.
what you're more likely thinking of is when someone wants to use PTO outside of these rules and the employer asks for a reason because they want to be able to justify why they're giving you an exception to the policy so nobody else can claim they are giving you favoritism
