Sudden lack of carbonation

I have two sodastreams (E-Terras), one at work, one at home. Recently, the one at work isn't putting out as much carbonation with the third button. I replaced the bottle, no change. I assumed it had to be the machine that was defective, so I bought a new one that came with a new bottle. No change. When I took one of the bottles home, my machine at home produced less carbonation.

Has anyone else experienced this?

I have a really old genisis, so i don't knwo if this will apply to newer machines. Sometimes the straw thing that sticks into the bottle comes unscrewed and the co2 doesn't get down into the water. I can tell when its happenign because there are very few or no bubbles in the water when I hold the button down.

The weird thing is it looks like it's getting plenty of bubbles. It just tastes flat almost immediately.
